Patient’s Rights and Responsibilities

Patient Bill of Rights

  • You have the right to safe, high quality medical care, without discrimination, that is compassionate and respects personal quality, values and beliefs.
  • You have the right to choose your healthcare provider.
  • You have the right to participate and make decisions about your care including refusing care to the extent permitted by law.
  •  You have the right to have your illness, and treatment and outcomes explained in a manner you can understand. You have the right to interpretation services if needed
  • You have the right to know the name and role of your healthcare providers.
  • You have the right to request that a family member, friend or provider be notified that you are under the care of this facility.
  • You have the right to be informed about transfers of care to another facility or and be provided complete explanation including alternatives to transfers.
  • You have the right to know the policies that affect your care and treatment.
  • You have the right to private and confidential treatments, communications and medical records to the extent permitted by law.
  • You have the right to be informed of charges and receive counseling on the availability of known financial recourses in healthcare.
  • You have the right to be free from restraints that are not medically required or are used inappropriately.
  • You have the right to access advocacy or protective service agencies and a right to be free from abuse.
  • You have the right to have your complaints, concerns and grievances addressed. Sharing your grievances will not compromise your access to care, treatments or services.

As a patient, you have the following responsibilities:

  • For arriving on time to your scheduled appointment
  • For meeting your financial obligation to the facility
  •  For providing accurate and complete information about your health status and, medical history
  • For providing complete and honest information regarding your insurance benefits
  •  For informing the doctor/provider or nurse if you do not understand the treatment, diagnosis or what you are supposed to do.
  •  For following the instructions and treatment plan that is advised by your practitioners/ providers, nurses and other medical personnel concerning your care.
  • For accepting consequences of refusing treatment or not following instructions.
  • For notifying the healthcare providers if you have an Advanced Medical Directive or similar document
  • For following Famcare’s rules and regulations
  • For acting in a manner that is respectful of other patients, staff, providers and facility property

Paying for Your Visit

Famcare participates in federal and state-funded programs that provide essential services to uninsured individuals and others on a sliding-fee scale, determined by the individual’s or family’s ability to pay. As part of these programs, patients may need to register for services and provide information to verify their identity, income, and dependents. This information helps determine eligibility for services and payment levels.

To qualify for discounted services and Presumptive Eligibility Medicaid Enrollment, you will need to provide proof of income.

Famcare accepts various types of insurance and offers assistance with Medicaid registration for eligible patients. No one will be denied services due to an inability to pay, and flexible payment plans are available.

Famcare’s Sliding Fee Discount Program

Famcare offers a Sliding Fee Discount Program that adjusts fees based on each patient’s ability to pay. The program applies a schedule of discounts to the standard fee schedule, taking into account the patient’s income and family size to determine their eligibility for reduced fees.
